Regarding marijuana and sleep, newcomers need to work up to the right dose gradually before any conclusions can be made, whether it helps sleep or not.  Then the amounts of THC and CBD, the main psychoactive and non-psychoactive components of most varieties of marijuana.  Experienced practitioners recommend 1:1 THC:CBD, while newcomers should start on 1:4 THC:CBD.  Oral dosage to start is only a few mg, and added a few mg per week until the minimal effective dose is found.  The vast majority of patients respond favorably under these conditions.  

Patients new to THC that take a large dose will probably not sleep the best, and can experience side effects like dizziness, coherent thought and couch lock (can't easily move).  Side effects are temporary in nature and marijuana has been shown to be extremely non-toxic.

 

 

COMMENTARY

 

The medical marijuana revolution reflects an information revolution.  Crazy statements from the government and the powers that be, no longer need to be tolerated, such as Jeff Sessions obtuse comment that "good people don't smoke marijuana."   Well, prior to the Fed declaring marijuana illegal in the 1930's, the American Medical Association protested against the Fed's scientifically unfounded decision to declare cannabis a narcotic.  Cannabis was listed in the American medical texts for more than one hundred years, including the bible of medicine, US Pharmacopeia.  In addition,cannabis based medicines were prescribed in nearly ALL pharmacies.

Technical note: d9THC or plain refers to that main psychoactive component of marijuana.  Technically, there are many kinds of THC, different isomers which have different properties ,some psycho active and some not.  Most varieties contain mostly d9THC and little else, but the trend is to breed cannabis abundant in other molecules that are medicinal but are non-psychoactive.

 

So called orthodox medicine criticizes the use of marijuana based on the "fact" that there is no scientific, clinical or other reputable evidence that it has medicinal benefits.   This claim is profoundly ignorant, and reflects the plight of the medical industry today.  Rather that being a public service, the focus is on patentable drugs and profits.  This is why doctors make about twenty times the average trained worker wage, and this is why the pharmaceutical industry takes in about 1$ trillion dollars per year across the planet.  

 

With this kind of money, big pharma hires machiavellian agents - CEOs, lawyers, lobbyists, propagandists, scientists  to pursue profits. This group has a stranglehold on information, the legal process, drug regulations, permits, education and law and order.   The reality is that most of American's life savings are sucked up into medical bills and their kids are left with, in most cases, no inheritance to speak of.  Such is the devolution of the me generation, the baby boomers.

However, the monopoly of information has been broken with a free and open internet. At least for now.  Google, Apple, Amazon, Facebook, and the rest are facing countless lawsuits for censoring and manipulating the data we see.  Under this framework, it is only a matter of time until we're back to square one, where the big corp and big governments controls all the information we see.

1 Functional role for cannabinoids in respiratory stability during sleep. Carley et-al 2002. Sleep 25: 399 - 400

This study demonstrates potent suppression of sleep-related apnea by both exogenous and endogenous cannabinoids. These conclusions are of relevance to the pathogenesis and pharmacological treatment of sleep-related breathing disorders

 

2 Intra Nodose ganglion injections of dronabinol attenuate serotonin-induced apnea in Sprague-Dawley rat. Calik et-al 2014. Respiratory, Physiology & Neurobiology 190: 20 - 24.

Dronabinol (synthetic THC) injected locally into the nodose ganglia suppresses five-HT-induced reflex apnea, and increases phasic, but not tonic, activation of the genioglossus. These data support the view that dronabinol stabilizes respiratory pattern and augments upper airway muscles by acting at the nodose ganglia. These conclusions underscore a therapeutic potential of dronabinol for the treatment of obstructive sleep apnea.

This concept study demonstrates that dronabinol (synthetic d9THC is quite safe, is well-tolerated, and reduces apnea and insomnia by approximately a third over three weeks of oral-administration. Dronabinol treatment might be a viable alternative or adjunctive (simultaneous) therapy in specific patients with OSA. It is important to reproduce these conclusions be replicated, in fully powered parallel group studies, to identify the effects of different doses and duration of dronabinol treatment in patients with O.S.A..

The American Academy of Sleep Medicine, researchers at the University of Illinois Department of Medicine reported “potent suppression” of sleep-related apnea in rats administered either exogenous or endogenous cannabinoids. Collectively, our data support the sleep promoting ability of WIN2 and may also explain some of the working/short-term memory deficits seen after administration of this drug as they have been related to hippocampal prefrontal interactions during active performance in learning tasks. 10 Cannabis and Sleep

The studies on cannabis and sleep that were conducted in the 1970’s (see Roehrs and Roth, 2011), give some information about the possible effects of cannabis on sleep. Low doses of d9-THC (4 - 20mg) mildly decreased REM sleep in both regular users and nonusers. Interestingly, deep sleep was increased when cannabis was initially used but this effect disappeared after repeated use. With high doses of d9-THC (50 - 210mg) REM sleep was decreased in both regular users and nonusers. Total sleep duration was not affected but deep sleep was decreased ,which is an important finding.. IF d9-THC was discontinued, some rebound in REM sleep, it was found, with reduced sleep duration and increased time to go to sleep.

12 How to Treat Insomnia With Medicinal Marijuana

A 1973 study on the effects of d9THC on sleep cycles showed that medicinal marijuana could in fact treat insomnia by decreasing sleep latency and increasing stage-3 sleep. A twenty mg dose of cannabis was shown to reduce the amount of time it took to fall asleep by an hour, though a higher 30mg dose was shown to make it more difficult to sleep. However, results also indicated that low and high doses of d9THC did slightly decrease REM sleep, and although it initially increased deep sleep, repeat use of marijuana decreased the effect. Another study in 1981 researched the effects of cannabidiol (CBD) and results indicated that patients who get one hundred sixty mg of CBD sleep for longer periods of time.

 

13 Therapeutic use of CBD and d9THC in Sleep

Therapeutic use of CBD may lie in its ability to counter some undesirable effects of d9THC (sedation, psychotropic effects, tachycardia), thus suggesting that if given together with d9THC, it may allow higher doses of d9THC.  d9THC and CBD have been administered as an oral mucosal spray to 58 patients with rheumatoid arthritis . Treated patients had significant reduction in pain and improvement in sleep compared to patients given placebo.
