Animal studies and preliminary clinical trials strongly indicate that cannabidiol (CBD) in concentrated ethanol extract form have antiseizure properties in children with treatment-resistant epilepsy. However, since 1932, cannabis based medicines are not (as yet) re-registered as medicines for prescription by American regulatory agencies.
In this study the cases of two children with treatment-resistant epilepsy is described. In both cases, these children did not respond any prescription medicine: phenytoin, topiramate, carbamazepine, levetiracetam, lamotrigine, primidone, and clobazam.
Case A has left frontal dysplasia
Case B has Dravet Syndrome
A 10-year-old girl was diagnosed with refractory epilepsy as a baby of 5 months of age. Prior to cannabinoid treatment she had approximately three complex focal seizures.
Treatment with phenytoin, topiramate, carbamazepine, levetiracetam, lamotrigine, primidone, and clobazam were ineffective, did not put seizures into remission. The patient underwent focal resection of the left anterior frontal lobe at age 8 (Figure 1) and was seizure-free for 4 months. She was dosed orally with CBD extract ( 208 mg/day or 6 mg/Kg/day; divided in three daily doses of 70 mg) with topiramate 1 mg/kg, clobazan 0.4 mg/kg and valproate 12.7 mg/kg.
This mixture eliminated seizures and improved general behavior, speech, understanding, and attention.
After 4 months she showed symptoms of psychological side effects, inappropriate laughter, ataxia, reduced attention, irritability, aggressiveness, spasms, bilateral mydriasis and eye redness
An analysis of the extract showed about 4%Δ9-THC and 90% CBD, while hair analysis confirmed the ration of THC:CBD. A 99.6 % purity CBD extract was substituted with no measurable Δ9-THC. Pure CBD extract oral showed no intoxication effect, as assessed by clinical evaluation and EEG results Pure CBD extract oral led to complete seizure remission after 4 weeks.

A 7-year-old boy had a diagnosis of refractory epilepsy due to SCN1A mutations associated with Dravet syndrome. He had daily complex, focal, myoclonic, tonic, absence seizures and severe myoclonic epilepsy,
He had substantial behavioral and developmental delays, hyperactivity and impulsiveness, and autistic-like symptoms. This boy was treated with anticonvulsant treatment stiripentol, valproic acid, oxcarbazepine, topiramate, levetiracetam, phenytoin, phenobarbital, and sulthiame.
The boy was then given CBD extract oral 250 mg or 12.5 mg/Kg divided into two daily doses (125mg CBD); in addition to topiramate 3.75 mg/KG and valproate 24 mg/kg daily.
Seizures then were reduced from 1–2 per day to once a month. General behavior, speech, understanding, and attention improved.
After 3 months, he suffered from psychological issues; ataxia, reduced attention, irritability, aggressiveness, and seizure worsening. Analysis of the extract used showed a residual of about 3% THC, with 91% CBD. Hair analysis confirmed the above THC:CBD ratios. A 99.6 purity CBD extract, oral 200mg (100mg 2x daily) without measurable THC was substituted and the trial resumed.
This dose was increased by 50% up to 300mg (150mg 2x daily). The deletion of THC led to a complete reduction of all intoxication symptoms. Complete seizure remission was achieved after three weeks. Autistic like symptoms were mostly eliminated. The boy joined in school activities, socializing, swimming, sports.
One year, both cases A and B were seizure free without intoxicating effects. Dosage of CBD and pharmaceuticals (described above) were maintained throughout.

Despite this, minor and infrequent side effects were reported."
